The following provides a summary of the information about the three medical plans we offer.

Take the time to understand how these plans work, the coverage each provides, and how to use them to best meet the needs of you and your family.

You have the choice of three medical plans:

  • PPO 1500/3000
  • PPO HSA 2000/4000
  • EPO HSA 4000/8000

Each plan has four coverage options:

  • Employee
  • Employee plus spouse/domestic partner
  • Employee plus child(ren)
  • Family coverage

Each plan has its own set of copays, deductibles, out-of-pocket maximums and coinsurance, and preventive care is covered at 100% when you use an in-network provider.

For the PPO and PPO HSA plans, you can use in-network and out-of-network providers. However, you will typically pay lower costs when you use in-network providers.

The PPO HSA and EPO HSA are HSA-eligible plans, meaning that you can contribute to a Health Savings Account (HSA).

For the PPO HSA Plan only, the company will contribute $500 when you enroll in individual coverage or $1,000 when you enroll in family coverage to your HSA.

Preventive Care

All three medical plans include 100% coverage for in-network preventive care. This includes:

Physician office services

  • Routine physical examinations
  • Well baby and well child care
  • Immunizations
  • Lab, X-ray or health screening tests*, including those for:
    • Mammography
    • Colorectal
    • Osteoporosis

* Many types of exams are subject to health status and age/gender guidelines.
